 and set up supportive service outposts catering to family caregivers. As of the end of 2021, the Ministry has established 114 service locations across 22 municipalities.
To make supportive services available to high-stress family caregivers early on, the MOHW related departments, NGO, and municipal governments devised a set of preliminary screening criteria as well as service transfer processes. These were announced on May 10, 2021, to assist all system network members to distinguish high-stress family caregivers and upgrade cross-sectoral referral resources efficiency. In addition, make sure services effectively respond to the needs of family caregivers. The MOHW demands municipal governments to establish training for family caregivers, empowerment and supervision mechanism, nurture network members' ability for identifying high-stress family members, to upgrade the quality of high-stress caregivers. In 2022, local health agency related assessment was required to be included in the internal unit horizontal contact cooperation and public- private partnership mechanism set by local government to ensure implementation of horizontal linkage at local level.
(2) Expand Respite Care for Families with Foreign In-home Care Workers
The MOHW and the Ministry of Labor jointly implemented "the Program for Expanding Respite Care Services for Families with Foreign In-home Care Workers". Beginning on December 1, 2020 recipients whose LTC grade 2 or higher can apply for respite care services when their foreign in-home care workers take time off to alleviate the stress and burden of family care workers without 30-day restriction. According to statistics, 17,225 people used respite care workers services with foreign care workers in 2021.
Compared to 2019, (2,791 people) there was growth of 6.17 times, and compared with 2020 (10,177 people) growth of 1.69 times. Use has grown over the years.
4. Family Physician Care Program for Home- Based Disabled Case
To establish integration between home-based care and long-term care services models so as to prevent patients of chronic diseases from deterioration and worsening in disability, the Ministry has implemented its "Family Physician Care Program for Home-Based Disabled Case" from July 19, 2019 onward. The Program involves dispatching nurses to the homes of disabled cases in nearby communities and perform health and chronic illness management, issue LTC physician's opinion and special precautions for service personnel intending to care recipients and so forth as the basis for the formulation of care plans. As of the end of 2021, a total of 880 clinics/health centers took part in the project.
5. Long-Term Care 2.0 Link Up Discharge Preparation Plan
In order to better help patients with need for long-term care services after their discharge from the hospital ASAP, the Ministry has established the "Incentive program for Long- term Care 2.0 link up discharge preparation plan" to integrate relevant tools of evaluation, personnel training, information system and evaluation processes so that patients can now have their long-term care need evaluation completed 3 days prior to their discharge and begin receiving their long-term care services as soon as within 7 days after their discharge.
